Sports Physicals Packets

Physicals information for the 2026-27 School Year!

Attention all incoming 2026-2027 students! Both Tomball High School and Tomball Memorial High School are offering physicals for student-athletes. This is a great opportunity to ensure you're cleared for participation in school sports.

Follow the instructions outlined below to ensure you have completed all necessary steps and forms:

1. Log in to the Tomball ISD Student Management Portal (RankOne). If you do not have a profile, you will need to create one.
2. Complete and electronically sign ALL 4 Digital Forms (pictured below)
   • Medical History
   • TISD Random Drug Testing Form
   • TISD Pre-participation Form
   • UIL Forms Signature Page

3. Download and Print the 2025-26 Tomball ISD Physical Form. No other form will be accepted. This form must be filled out and completed by yourself and the physician performing the physical.
4. Select and purchase your appointment time below. Ensure you uncheck the "myself" box and type the First and Last Name of the student getting the physical (no nicknames) in the box provided.

5. Print, complete, and sign the Houston Methodist Permit Form.
6. Arrive 10 minutes prior to your appointment time for check-in:
   • Tomball High: Lower Lobby Parking Lot
   • Tomball Memorial: Athletics Parking Lot
7. Bring the completed forms:
   • Houston Methodist Permit Form
   • Tomball ISD Physical Form
   • Proof of Appointment/Physical Purchase
8. High School students should turn in their completed Tomball ISD Physical Form to the Campus Athletic Trainers. Junior High students should turn in their completed form to their respective coach.
