Sports Physicals Packets
Every Student-Athlete must submit a new TISD Physical and all RankOne Digial Forms for the 2026-2027 school year.
Follow the instructions outlined below to ensure you have completed all necessary steps and forms:
- Log in to the Tomball ISD Student Management Portal (RankOne). If you do not have a profile, you will need to create one.
- Complete and electronically sign ALL 4 Digital Forms (pictured below)
- Medical History
- TISD Random Drug Testing Form
- TISD Pre-participation Form
- UIL Forms Signature Page
RankOne portal
- Download and Print the 2026-27 Tomball ISD Physical Form. No other form will be accepted. Accuracy on this form is important! Please read and answer all questions honestly and sign at the bottom of the Medical History page. The Healthcare Provider will complete the Physical Examination page and should sign/stamp and date the form.
- High school students should turn in their completed Tomball ISD Physical Form to their campus athletic trainers. Junior high students should turn their completed form into their respective team coach. Digital copies are accepted via e-mail as long as all blanks are legibly filled in and the physical examination is signed, stamped, and dated by the healthcare provider.
